A Case of De Novo Cyclosporine A Associated Hemolytic Uremic Syndrome in Renal Transplant Recipients

Abstract
The cyclosporine A associated hemolytic uremic syndrome has been reported previously in recipients of bone marrow, cardiac, liver and renal allografts as a dramatic but relatively rare disorder. This syndrome, accompanied by platelet fibrin thrombi in glomerular capillaries, is likely to occur when the plasma levels of cyclosporine A are high and during the first weeks after transplantation. The outcome is poor. Some therapies effective in other forms of hemolytic uremic syndrome have been tried, but only a minority of patients recovered renal function while most lost their graft in a matter of weeks. Early recognition of the syndrome and discontinuation of cyclosporine A has occasionally led to reversal of the syndrome.
We report a case of de novo cyclosporine A associated hemolytic uremic syndrome occurring within first week after kidney retransplantation, in which the patient was successfully treated with decreasing the dose of cyclosporine A and intensive plasma exchange.
